What to Expect from a Comprehensive Exam

Your first visit to our office will include a comprehensive dental exam so that we may create the best treatment plan for you. This appointment will include a dental decay exam, periodontal evaluation, oral cancer screening, and any necessary digital x-rays. Diagnostic model impressions and photographs will be taken if necessary. Each visit will include a thorough medical history review so we ask that you bring any medication lists you may have. We will then explain to you the conditions present and the best plan of treatment. With your approval, we will then discuss estimated fees with insurance coordination, and schedule your well-planned appointments.

Patient Forms

If you are a new patient to our office, the attached files contain our new patient forms that will need to be filled out when you arrive at our Benton dental office. Printing them, filling them out and bringing them with you will allow us to attend to you more quickly than completing them on your arrival. Insurance and Financing

If you have dental insurance we will be glad to help you determine your coverage and maximize your benefits. We ask that you assign your benefits to us and pay deductibles and co-payments at the time of your treatment. Personal payments may be made with cash, check, and credit card. We also participate in the Care Credit program that may allow limited interest-free payment options for you.
